Essential duties include:

• Conduct assessments on patients
• Develop and implement the hospital's plan of care
• Educate patient and family on the plan of care
• Maintain accurate and updated medical records on patients

Qualifications:

• Current Registered Nurse (RN), Licensed Practical Nurse (LPN) or Social Worker (MSW) in the State of Louisiana
• Minimum of 2 years experience in an inpatient rehab or skilled nursing facility
• Compassionate team member with time management skills
