Duties:

• Provides physical therapy services, reassessing the patient’s status during each treatment and ensuring quality of care as evidenced in the clinical record.
• Develops, discusses and communicates a plan of care for each patient, in collaboration with each patient/family/significant other in order to address all identified needs.
• Maintains up-to- date and accurate documentation of physical therapy services provided to ensure the integration of information for use by the healthcare team, to ensure quality care.
• Complete appropriate discharge planning in a timely manner.

Requirements:

• Current Texas Physical Therapy License
• Doctorate Degree (DPT) from a CAPTE- accredited physical therapy program
• Two years of experience, long term acute care and rehabilitation experience is preferred
